Rates
$185 per Intake evaluation/Clinical interview (60 mins) .
$160 per individual session (50 to 60 mins).
$175 per hour for psychological testing.
Insurance
If I have a contract with your managed care company the billing procedures of that company will be followed and you will be expected to pay the co-payment, which is the amount not covered by your insurance company, at the time service is provided to you.
Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:
- Do I have mental health insurance benefits?
- What is my deductible and has it been met?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?
- Do I have an Out-of-Network benefit?
Payment
Cash, check and all major credit cards accepted for payment.
Copayments, deductibles, coinsurance, or any other fees are due at the time of your appointment.
Cancellation Policy
If you do not show up for your scheduled therapy appointment, and you have not notified us at least 48 hours in advance, you will be required to pay a $75.00 missed appointment or cancellation fee.
